This is the pattern that gets missed most often — and it gets missed because it requires a clinician to look outside the body for the cause, not just inside it.

Your mast cells are not overreacting randomly. They are responding to a persistent immune stimulus that has not been removed. For Pattern 3, that stimulus is most commonly one of two things: mycotoxin accumulation from mold exposure (current or past), or chronic viral reactivation — most often Epstein-Barr virus, HHV-6, or cytomegalovirus. In many cases, it’s both.
